Can someone tell me what mod this is?

ms_vopt_permissions

It is causing me a database error.

I have disabled all of my plugins, but yet it still causes the error.

Putting "ms_vopt_permissions" into the search bar gives me this thread with the solution - Database error: Password Expiry/History

If you have uninstalled this modification, make sure you remove its files (especially the ones in /includes/xml). A rough guess on where the error occurs is inside the Admin CP.
