I want to be able to allow users to fill out a form, and have it auto posted to a ceartin forum. Then I would like to go into the admin panel and approve or deny this form and have a drop down menu that would be like "Approve-Access 1", "Approve Access 2", "Deny".

Has anybody seen anything like this before?

I have not seen anything like that, no. There is a mod to create a form that may be used to create a thread, but the rest of the stuff about approving access is not part of the form modification.

Why don't you just turn on moderate new members in Admincp-user registration options. The questions you want to ask in your form, use custom profile fields. Then when revuewing members awaiting moderation, you can read their profile and assign secondary usergroups for acces to 1, 2, etc... Then approve, or deny thtm?
