Can someone point me towards a tutorial or example of creating a data entry form for use within vBulletin please?

I'm writing a product for vBulletin within which I need to allow users to fill in a form and then I store that data in the database.

Look in the Mods section. Search for a thread with "form" in the title. I know I've seen a hack in there for it.
