How do I add a variable form input

Yeah so I'd like the entry in one of the fields on the form to be one of the forums names.

It is for a password request mod and I want it so that when the users see's the enter password page there is a link that takes them to a request form. On the form I'd like 'The forum you wish to access:' to be pre-entered.

How do I do this?
