Looking to add a store locator to my site

I am looking to add the ability to add a store locator to my site for members to add / edit and search for sport card hobby stores in a given area.

I have some structure built as a custom database, but I lack the XML/PHP knowledge to hook it all together.


Some of the fields that I would be looking to capture

• Store Name
• Address (with the ability to trigger a google map at the bottom of the page
• City/State or Province/Postal Code
• Hours of Operation
• Store URL
• eBay URL (I do have an automated eBay Link rewriting mod)
• Option for up to 3 other online store interfaces that I can define
• Information about the store
• I would also like to capture who made the last modification on the page.

I am not sure if something already exists with the majority of this options; but it is does not this would be something I would be willing to pay a bit for the development.
