For my CMS frontpage I'd like the following "setup".

featured
advert
X|X|X
X|X|X
X|X|X

Featured is a flagged article that always comes on top. (replaced when a new featured is made) X represents articles from all subcat's (2 columns (or 3 if needed))

Is this possible?

I doubt it can be easily done at this point. There are no Ad templates defined for CMS as far as i remember (might be wrong) But inbetween articles through structure might not be easily possible right now, Perhaps some serious cms templates edit can achieve it but im guessin this will be addressed in coming release.
