Second 'Structured' Signature

I need a, somewhat specific, mod. First off, it would create a second signature box below the first one. Then, it would have:

(USERNAME)'s Portfolio

Thread 1
Thread 2
Thread 3

The threads are selectable by the user. In any thread that the user started, there would be a text link that says 'Add to Portfolio.' They can select which threads to add to here.
