How do I remove this template?

Within my main template is one called "quickreply_addon"

I want to delete this, but can't. I removed the product already (I'd like to use it but it's just not working for some reason), but it's still there.

I can't "Revert" it because the option to do so is grayed out. How on earth do I get rid of it?
