The + sign indicates members who are in your friends list on vBulletin. The asterik(*) indicates people who are invisible...or appear offline as you might want to call it.

The plus symbol means they are friends on your Contact List. The * symbol means they are invisible. Edit: Lol, Mahar beat me by a split few seconds |

I believe there is an option in the vBulletin Options to disable the Who's Online feature if you want just the + signs etc to disappear you'll have to edit some templates
