How do I include a .php file in my header template?

Hi,

I'm trying to include a sitewide header file 'head.php' in my header template as a server side include but nothing is happening, I've looked into it and as far as I can make out I have to use a 'hook' however I can find no information on hooks - I'm using 3.6.5
Any help please?
