I see on many forums that I visit that they have both a Captcha Image + a security question. I have only been able to choose one of the two. How can I have both measures active?

You would add a new User Profile Field and make it required at registration. I believe this is covered in the manual or else in the help for spam thread over on vb.com

Im not too clear on this, add a new user profile field? can you layout the steps please?

I know how to create a custom profile field but how do I incorporate the Captcha in addition to my Q&A?

I use this modification to specify Q&A along with my Captcha. It comes with some other features as well.
