How To Make A Custom Start Thread Page?

I Was Wondering,

How Would I Go About Making A Start Thread Page That Would Allow Default Sections?

Example:

When You Click New Thread In A Catagory That Is Specified By Admin,

You Would Enter The Thread Title

Then There Would Be Feilds That Are There That Must Be Filled Out Kinda Like A Application But Just For A Post

So When You Go To Create Thread You Would Have To Enter Lets Say,

1) Game Name

2) Times Played

3) Online Name

4) Favorite Map

Ect. Ect. Ect.

Dose Anyone Know What Im Talking About

And Know How To Do It Or Give Me A Link So I Can Learn How To Do It?

Cause Ya I Tried Looking But No Luck So Far.
