I am curious if there is any mods out there that would change the "Contact Us" button to a simple Trouble ticket system...

What I would like is a simple form that a Member OR guest could fill out and submit. When the form is submitted, I would like it to posted to a normal forum on my website where Admins, Super Mods, and Mods would all be able to access it...

Each ticket should be a new thread in the forum with a title like "Trouble Ticket - some text the user or guest enters as a subject". The form itself should include Name, UserName, E-mail, Phone, a couple drop downs for type of problem (registration, accessing site, member problem, report problem post, other), and a free text area... I would like to see this output in the new thread as a simple "item: Member Entered Info" with the free text below that.

The reason I want this is ofter I get e-mails through the contact us form that any member of the staff could have addressed. This way there is no e-mails to get lost and after a problem is addressed the new thread can be closed or deleted.

I don't know how difficult of a mod this would be for some of the more experienced coders but conceptually it would not need to be too complicated and I assume many other forums could use something like this.
