As to the first question, the short answer is yes. The longer is answer is that while it could be done, no specific hack exists as of yet, so to do such a thing you'd need to hire a vBulletin coder (this board has many) who could code such a feature for you. I'd suggest if you do want to do that, you PM somebody with a "Master Coder" label, as they'd be mostly likely to be able to help you there. Or, occasionally the owner of vbadvanced does custom work, so he could probably do it at a reasonable price for you too. As to number two, a survey form itself is very simple.
