hmm, then maybe it's another hack then. Geek did address the form cleared problem in either this or the install thread, as I had a problem with text not showing up for the checkboxes.
Just the first thing I would look at, as mentioned earlier, there are a couple other hacks that cause problems, or are set wrong in admin. other than check for those, like in the forum manager settings for that board, can't really suggest anything else. When Geek gets back (I think wednesday) maybe he will know.
