You could use public usergroups. Since it's basically impossible to ACTUALLY prove whether or not a person is male or female, it doesn't really matter. Create two public usergroups; one for men and one for women. Then tell users to join the corresponding usergroup.

Otherwise... there is a hack that you can use that puts users into a usergroup based on the answer they give to a custom field at registration.
