This has been mentioned a number of times over time but just to cover it again - technically it's user error, though some may say it's a flashchat 'issue' - it happens because you are not actually logging out of flashchat, but just closing the browser. Always log out using the red (top right) logout button. If you just close the browser then flashchat will try and run a pop-up to log you out - but browsers other than IE (such as firefox) block this.
