Well, there's actually 4 templates mod. Any of them should work with this template mod well. Just follow the instructions carefully and they should work. If you have trouble with a particular section, post it here.

Well, it depends what you have on your forum home page. If you have no hacks, then you could include what's in the instructions as well as the following:

Anything else would probably depend on a hack, which of course is not supported here. Check vBulletin.org for hacks.
