I'm hoping that this mod exists somewhere, and I just can't find it.

My forum is specific to members of a fitness center. There are disclaimers everywhere that we are not affiliated with or supported by that fitness center. I modified the feedback form so that the title spells that out in bold, but we continue to get questions about the company itself from guests. We send a fake 'auto-reply', but it's kind of a pain. I'd like the users and guests to have to stop and think.

So, what I'm hoping exists, is some sort of review form that pops up after the users submit the feedback. It would have the text displayed and some sort of agreement that this fits the criteria for feedback. Then the option to Agree, continue, or Cancel. At that point, the message would be sent.

Is there something like that around here?
Does this make any sense?
