Adding "report this thread" beside the post reply button

How do I add "report this thread to a moderator" beisde the "Post Reply" button thats on top of this post?

and getting it to send to the mods like the report post does?

I cant even figure out what template to put it in, nor what the code should be so that it sends to the mods.
