Do NOT change the custom user title via the Admin CP. Each member must change it in their User CP. That's your problem.

Also, make sure you have switched this hack to ON in your Admin CP, vB Options, Custom User Title section. When you first install this hack, the default it OFF. And that you have not restricted this hack to Admin only.
