Click Install for all the hacks you use here and vB.org. And/or create a template which has a list in them, and/or make a thread in a private forum listing all the changes you have made that arn't part of a hack. And comment all changes in PHP files so when you upgrade you can use Agraxis Merge to easily update your forums.
