Wooo hoooo! I have figured out the who's online problem. Apparently is is a vbulletin bug.

If your private forums are inheriting their permissions from a parent forum, then the thread titles will show up in who's online -- though unauthorized users will not be able to access the thread.

To fix the problem, simply use custom permission settings on each of your private forums. I did this on my board and tested it. It worked! Needless to say, I am quite happy.
