vbulletin 3 already has built in multi poll, you can add multiple options per poll as answers to the question.

Better reading of your question in the op, the answer is actually no.

You would need a custom hack/addon to have a single poll with more than one question in a single poll.

I dont recall ever seeing a addon like that.
