You should reply to your original post about this, not create a new one.

But, now that I can see your link, I would suggest this:
1. Create a TOS page in CMPS (use PHP page and link it to your current page)
2. Create a reports page in CMPS (use PHP page and link it to your current page)
3. Edit the PHP code on the TOS page so that the form submit goes to the new Reports page instead of the old one

Or you can take the code from each (current) page and create a new PHP Module from them. Using PHP pages instead of Modules would be easiest, but some of the php code can conflict, make sure you check the option to Clean File Output.
