I'm having the exact same issues as shown in your screenshots. Im on vBulletin 4.2.3, fresh install of dbgallery, imported from photopost vbgallery 3.02.

There is, and it shows up for me, however it shows these images at full size so I can't use it. I tried changing the Instance thumbnail settings and max image settings but no change.

Any suggestions for the above issues?
