To fix the text overlapping the image, in the ozzmodz_t_closed_reason.css find the following:

#ct_image {
    position:absolute;
    padding-left:2px;
    width:125px;
    height:125px;
    float:left;
}

And remove the line:

position:absolute;

I think I had that in there when I first added the image.
