Thank you very much!
I have installed the attached link, all seemed to work smoothly i create new page to experiment with the module and then when i finally see the new menu on the AdminCP 'Application Forms' Somehow i click either one of the options (Application Form Settings, Types, Positions, Edit default Question) and all get is blank page with no data or menu, just white.

Just when i felt i finally made it....
What could i have done wrong in the process?
