It does demote inactive users. That's the whole point of the mod. Personally I use it to move 'Users Awaiting Email Confirmation' to a limited access usergroup if they don't confirm their email address within 15 days of registration. And it has worked perfectly since I wrote the mod.

What version of vB are you running?

Did you select the proper demotion type (primary or additional)?

Did you let it run for at least 1 hour? Demotions are done by scheduled task and if you don't have enough visits to your site, it could take longer.
