Thank you very much! I was able to enter it! The problem was the value of the page kalleis wrong (it's called "banner").
Instead I can not do the same thing in the pages of the form of Easy Form
The value of the form.php file is "forms".
An example page is this:

I entered:
forms | forms
forms | forms.php
forms | Easy Forms
forms | forms.php? do = form & fid = 1

But it does not appear. : (Any suggestions?
