If your forum has never been vBulletin 4.x, how come you have a file from vBulletin 4.0.2 uploaded to your /includes folder?

You may want to go through the results of the "Suspect File Versions" in the Diagnostics page under the "Maintenance" AdminCP group and ensure you know what each of the files that aren't recognised as being part of vBulletin 3
