Do you have admins who are able to install things without your knowledge? This one doesn't require any file uploads, but it does require one PHP edit. The developer has made a version of this mod for every version of vB starting with 3.6 going all the way to up to 4.x. So if you've been vBulletin-ing for a long time, it's possible you just forgot about this one.
