Do you know if the owner is still active? It might be easier for him / her to implement an option to choose what format you want.

As an example, after asking if it was possible, instead of having us do server work to change it, DragonByte Tech recently implemented a format option for their Username Change modification where you type in the format you want (e.g., n/j/Y - 1/8/2013) in a type box, which controls the format of the Username History tab on profiles.
