How does that happen? It's not designed to catch human spammers, therefore if a spam account gets through, is it human? Is it a fail?
To your list I would add
Ban Spiders by User Agent and although it's not specifically a anti-spam mod, it's very handy in that arena. For example, if you ban all the early IE such is IE 2-6, you wipe out a big percentage of the autospam right there...
I also use
Hostname or user agent registration ban to stop spam registrations from the amazonaws, yandex, and many other host names.
