i am looking for someone to design the following

Entry form for the following

Username:
Profile Link :
Article/newspaper link:
Order type:
- Votes
- Subs
- Likes

Number of votes/subs/likes:

after the click Make an Order

is should then redirect to page 2

with links of who to pay and how much
and at the bottom a confirm of order to complete the process



now the backend
Choose usergroups who have the right to change the price of
likes
votes
subs

the system should be able to show


and at the bottom



the users End

1) each user to have own refferal link
2) if user orders from that $cookie ( refferal link ) they get xxxx amount added per accepted order


admin/mods end
each order that is accepted by them they get commision lets say 10% of that cost of order
