When I create an album, it automatically goes to the default "uncategorized" category. There is no option to choose a category. In admin CP, I can see the albums, but when I click on an album to edit, there is still no option to change the category.

I'm sure I've probably just missed a simple tweak. Any ideas please?

Read the installation instructions at first post. Are you sure you have done the part three:

3. (Important) Open template album_edit and find:
