ever since i upgraded to 4.1.10(or .09 not sure the exact time it started happen)

my forum reply box is acting up

here is what it suppose to look like
(this is the only place[report section] it works fine with background the highlight color)

here is what all my reply box/ quick edit /post thread look like
its really really annoying and hard to read

i have 4 templates, all the same problem

i have tried disabled all plugin hooks, same problem

any idea?
