If you go to Users > Prune and choose the "Users Awaiting Email Confirmation" from the drop down box you get them all on one screen and can delete the whole lot in one go. You shouldn't be getting just 20 at a time.

In fact, if you have thousands you might want to filter that down a bit to smaller chunks via using the from and to date option.

They don't delete automatically AFAIK
