Hi, I'm looking for a template or outline that can be used in a single Category that requires members to fill out all fields before a thread can be created in the Category.

Preferably something with customization available as to how many fields and what those fields are. As an example this is one layout I'd like to be able to set up:
Make:
Model:
Caliber:
Location (city or county):
Price:
Will ship (Y/N):
Other info:

Does such a thing exist or could it be made?
