I'm going to guess that you're talking about user titles present in the User Title Manager. Sorry if I'm wrong

Two things:

1. To my knowledge: promotions are not in real time. They are handled by the promotion script every hour (unless modified). This can be viewed in Scheduled Tasks.

2. Usergroup Titles and User Custom User Titles overrule User Titles. Make sure that your test user does not have a Custom User Title or a Usergroup Title set.
