We use Q/A and get probably 5-10 spammer registrations a day, I'm almost certain they are humans and not bots. I don't think there's a lot you can do about - it's a trade-off between making it easy to register on your forum and keeping out spam.

We have GlowHost Spam-O-Matic installed (the link is below), and that keeps out some by looking them up at StopForumSpam before allowing registration.
