Normally, you would use 1 placeholder for any kind of banner in your pages. This gives you any imaginable number of locations.

The positions 0...9 are merely types of banners, such as horizontal and vertical, although this is not entirely true. It is up to you how to use locations, my tip is to use as little as possible - most of the times, position 0 is just fine, for 1 or 100 banners on your page
