Any ideas of the DB error ?

The video retrieves information and all works etc but when you "submit" it comes up with the following

but submits it ok ........... in other words you go into the site by either refresh or back to site and the vidoe has been added it just gives the db error on submitting thats all instead of returning to the page etc
