I want to add an additional profile for new registration

ACP -> User Profile Field Manager -> Add New User Profile Field

my choice is

Single-Selection Radio Button

but I have no idea what to fill in

all I require is a check box beside "I am human and not a spanner"

and new registrants are required to click on the check box to finalise the registration
