Are you sure this is related? Because it's been installed on hundreds of 4.0.x installations and nobody has reported this issue.

Quite frankly I don't see how it's possible unless you use some form of modification that interacts with Avatars, and vBActivity is conflicting with it in some way.
vBActivity makes absolutely no changes to user avatars or their display mechanism.

It's working fine on all versions other than 4.0.1, which has a bug in the profile tabs system that renders any custom modification (including vB Blogs) unable to display profile tabs
