It is mainly bug and security issues fixex. Plus some new things added like the facebook connect.

Yes, it is advisable to upgrade your forum as it is always best to stay up to dated with the latest version, security-wise. Upgrading is easy. Have a look at the readme folder inside the vb package or at the online manual.
