I have the same problem. vBulltein 4.0.2 PL1, fresh copy, not upgrade.

Here are my PHP settings from the Diagnostic tool :

file_uploads: On
open_basedir: None
safe_mode: Off
upload_tmp_dir: None
upload_max_filesize: 40.00 MB

Response : "No errors occurred while opening the uploaded file for reading."

But i still get the stupid red icon when i try to upload files from my computer.
If i upload files by URL it works just fine. I do not understand.

Found a solution for my problem. I have disabled all Ajax features and now at least my Attachments manager works.

Admin Panel -> Settings -> Options -> General Settings -> Disable AJAX Features
