Make sure your using the latest version of vbulletin

In the left column of the admin control panel - vbulletin options drop down - human verification options - either use image verification, question and answer or setup ReCaptcha.

If you are using the standard captcha phrase, the spam bots broke that a long time ago.

I think there is a modification where you can use 2 or 3 of the human verification options at the same time.
