I just use a regular forum and set replies to moderated for that forum only (then never approve them). It doesn't have the columns you asked for, but I leave that to the OP to clarify (plus I discourage posting of email, phone, etc., so they use the forum email for contacting). I've considered restricting new threads to those in a "professional" category, but so far this is working fine.
