I've seen instances where Player B will accept Player A's challenge and if Player B wins, Player A won't confirm that they lost.

The player encountering problems should click the report image

on the right hand side of the challenge. Then you (or any other ladder admins) can confirm the score. There is option for player reporting the match to provide a screenshot as proof of score.
